Summary :
Introduction: The regional health agency of Nouvelle Aquitaine (France) launched a telemedicine program in 2021 for medico-social establishments (EMS) catering for adults and children with disabilities. Within this framework, the Esquirol Hospital in Limoges offered teleconsultations in Physical and Rehabilitation Medicine.
Objectives: The primary objective of this study was to identify the indications for telemedicine in PRM for people in medical-social institutions. Secondary objectives were to describe the clinical profile of the patients, to specify the mode of access and the teleconsultation process, and to collect the recommendations made.
Method: A retrospective study based on the analysis of data all patients included in the program between January 2022 and March 2024. Results: 26 teleconsultations were carried out for 19 patients, with an average appointment time of 25 days. The majority of patients were young men, with severe motor disabilities (tetraplegia or tetraparesis in 73.6 % of cases) and associated disorders, mainly related to cerebral palsy (80 % of cases). The main reasons for consultation were : device renewal (84 %), positioning assessment (76 %), pain (24 %), spasticity (20 %), nutrition (20 %), skin condition (12 %). It leads to an average of four recommendations for each teleconsultation. On the requesting side, occupational therapists were present in 91% of consultations and physiotherapists in 29%.
Discussion-Conclusion: Telemedicine in EMS offers new access to PRM care. Our results show that, as a complement to face-to-face consultations, and accompanied by an occupational therapist, telemedicine provides a rapid response to the needs of patients
with severe disabilities.
